Collecting data is a way that allows you to report your performance accurately…. Choose the measures you want to report making sure to include six (6) with an Outcome or High Priority measure to maximize scoring. Contact Covisint sales support if you need assistance on measure selection.
Review the specifications for each measure selected ensuring that you understand what is required to “meet performance”. Data elements should be collected in a unique data field for easy aggregation. Fields can be numeric or text depending on the specification. The text could be Y/N, Done/Not Done, Complete/Incomplete or some other equivalent. The key is that it is always the same for a given measure.
Be sure you also document any follow up, if required by the specification, in a separate field. For example, the smoking measure indicates if the patient answers yes they are a smoker that you provide smoking cessation counseling. You, therefore, need to document if they are a smoker in one field and did you provide counseling, if applicable, in a separate field.
Appropriate documentation… confirming that you are documenting appropriately and have the ability to gather the information required to report on the measures selected. Refer to the Covisint IM reporting instructions document for more information.
